Congratulations to the Rehab Model team in Cardiff & Vale UHB for winning the Value Based Care award in the Advancing Healthcare Awards UK 2023.
The recovery-focused rehabilitation model aims to optimise health and support self-management, focusing on addressing health inequalities, through a growing range of co-produced and co-delivered community interventions available in multiple formats. Together, the team are providing non-medicalised, health-generating care, delivered closer to home with the transition to sustainable long-term community-led solutions.
CEDAR is supporting this groundbreaking work by evaluating the effectiveness of two programmes in the Rehabilitation Model: Living Well and Prepare Well (Orthopaedics). This involves patient and staff interviews on their experiences of each service, as well as an economic analysis of the value of each service. Find out more HERE.
